When people first become eligible for Medicare, they often hear that a Medicare supplemental plan sometimes called Medigap is a “no-brainer.” These plans can fill in the coverage gaps that Original Medicare leaves behind, providing peace of mind and reducing out-of-pocket costs. But the truth is, supplemental plans are not the right choice for everyone. There are specific circumstances where choosing one may not make financial or practical sense. Let’s walk through three key situations where you may want to think twice.

1. If You Qualify for Both Medicare and Medicaid

If you’re what’s known as “dual eligible” meaning you qualify for both Medicare and Medicaid you do not need a Medicare supplemental plan. Medicaid typically pays first, covering many costs that a supplemental plan would otherwise pick up. In fact, there are specific Medicare Advantage plans designed for dual-eligible individuals, called DSNPs (Dual Special Needs Plans), which offer very comprehensive benefits. Paying extra for a supplemental plan in this situation is unnecessary.

2. If You Can’t Qualify Due to Health Conditions

Not everyone can easily sign up for a Medicare supplemental plan. If you try to switch from a Medicare Advantage plan to a supplemental plan outside your initial six-month open enrollment period, you may face medical underwriting. This means answering 35–40 health questions, and if you have certain pre-existing conditions, your application may be denied. For those who want a supplemental plan, the best time to enroll is during that initial six-month window when you are guaranteed acceptance, regardless of health status.

3. If Supplemental Plans Don’t Fit Your Budget

Cost is one of the biggest factors in deciding whether a supplemental plan is right for you. Currently, the standard Medicare Part B premium is about $185 per month. On top of that, most supplemental plans cost an additional $100–$150 per month. That means your total monthly costs could range from $285 to $360, not including drug coverage. If those premiums are simply out of reach, Medicare Advantage plans may be a more practical option. Many Advantage plans have lower or even zero monthly premiums and often include prescription drug coverage, making them a more affordable choice for people on a tight budget.

The Bottom Line

While Medicare supplemental plans can be incredibly valuable, they aren’t a one-size-fits-all solution. Dual-eligible individuals, those who cannot medically qualify, or those for whom premiums are unaffordable may be better served by Medicare Advantage or other coverage options. Personally, I prefer supplemental plans when they fit a client’s needs and finances but it’s important to understand that they aren’t the best fit for everyone.
