Enrolling in a C-SNP is simpler than people expect. You don’t have to wait for the Annual Enrollment Period. You can enroll any time of year as long as you have a qualifying diagnosis. Your doctor simply provides written verification of your chronic condition, and that paperwork must be submitted within 60 days of enrollment. Many C-SNPs also come with a $0 monthly premium, making them one of the most cost-effective options available to anyone with chronic disease.
